Connexion is an out-of-network provider.
What does it mean to be out of network?
In order to deliver high quality care, Dr. Mel has opted to remain an out of network provider. This means her practice is a fee for service model. Your card will be ran following each visit and an invoice (superbill) will be available to you, should you wish to submit to your insurance for reimbursement. do you accept insurance?
can i use my hsa/fsa/vba benefits?
Yes! Any specific information needed for your FSA/HSA/VBA claims can be easily accessed via your superbill.
Do i need a doctors referral to see you?
Oregon is a direct access state. This means that you are allowed to see a physical therapist directly without a referral. However, if you are utilizing out of network benefits for reimbursement, you may need to check with your insurance to verify if they require it.
Please give at least 24 hours' notice if you need to cancel your appointment to avoid a cancellation fee. A one-time grace fee of $50 will apply for the first cancellation. Repeat occurences will require you to pay the full session fee. We appreciate your consideration for Dr. Mel’s time and other patients who also need appointments.
